Clinical Manifestation of Secondary Headache Attributed to Intracranial Disorder with Only Acute Mild Headache

Abstract
Background: Acute mild headache patients have possibility of secondary headache attributed to intracranial disorder. These patients are sometimes neglected at out-patient department because of their mild symptoms. We sought to observe clinical manifestation of secondary headache attributed to intracranial disorder.

Methods: We enrolled patients who had acute but mild headache from March 2017 to October 2017 at Seoul medical center and were performed with Brain CT and MRI as evaluation of headache on the day of visit. Patients who have headache and intracranial disorder were analyzed.

Results: Total 240 patients were enrolled and 26 patients have intracranial disorder. Ischemic stroke, hemorrhagic stroke and brain tumor were diagnosed as 15, 8, and 3, respectively. Headache patients with ischemic stroke were younger and had short duration of pain than other groups (p=0.019). They were more likely to have throbbing nature headache (p=0.076) and neurologic deficit (p=0.078) but there was no significant statistical difference.

Conclusion: Even patient with mild headache, acute headache might be considered as secondary headache and they should be evaluated for differentiating primary headache and intracranial disorder including ischemic stroke.
